        This form must be returned to the High School Principal’s office 3 days

         prior to event.

This form must be completed to request permission for persons not attending Harmony Area School District to attend the dance.  Everyone attending a Harmony Area School District function must conduct his/her self in a positive and respectful manner.  School rules MUST be followed and anyone not following these rules will be asked to leave and no refund will be granted.
